A Closer Look at Your Vision - Initial Assessment Questionnaire

01
Your Glasses and Daily Routine

How many pairs of glasses do you use throughout the day, and when was the last time you experienced continuous, comfortable vision at every distance?

A
I use one pair of glasses, or no glasses at all. I see very well both up close and at a distance, without strain or having to think about it.
B
I’m constantly switching between different pairs - one for reading, another for the computer, and a third for driving. All this switching is very tiring.
C
I’ve tried multifocal glasses before, but had significant difficulty adapting to them - including dizziness, instability or distortion at the sides — and eventually gave up.
02
Screen Use and Eye Comfort

How do your eyes feel after 3–4 continuous hours of working on a computer or using your phone?

A
Comfortable and refreshed. I don’t experience dryness, fatigue or an unusual need to blink.
B
They burn, feel irritated and begin to feel gritty, almost like there’s sand in my eyes. Eye drops or artificial tears provide only temporary relief.
C
My vision becomes blurred or double. I blink and rub my eyes frequently, and sometimes close one eye in order to focus on text or an image.
03
Visual Acuity and Optical Focus

My vision becomes blurred or double. I blink and rub my eyes frequently, and sometimes close one eye in order to focus on text or an image.

A
A sharp, stable point of light, clearly focused in one place.
B
The light appears smeared, with “rays” or a halo around it. Words on the screen may appear with a slight shadow or double image due to astigmatism.
C
The light appears significantly distorted or stretched, sometimes almost cone-shaped. My vision is very blurred and unstable, even while wearing my current glasses.
04
Your Experience with Contact Lenses

What have you been told in the past, or what have you experienced yourself, regarding your ability to wear contact lenses?

A
I can wear standard contact lenses comfortably throughout the day, or I have never tried them and do not want to wear contact lenses.
B
I’ve tried contact lenses, but they made my eyes feel dry, became uncomfortable after a short time, or moved around and blurred my vision.
C
I’ve been told by eye-care professionals that because of my corneal structure, high astigmatism or dry eye, contact lenses simply aren’t suitable for me.
05
Visual Strain and Posture at the End of the Day

How do your head, neck and shoulders feel at the end of a focused workday?

A
Loose and light. I don’t experience any unusual pain or tension around my face or eye sockets.
B
I often experience a dull headache across my forehead or around my eyes, and regularly feel the need to massage my temples or the bridge of my nose.
C
I find myself tilting my head at unusual angles, stretching my neck forward toward the screen, or experiencing significant physical fatigue directly caused by the effort to focus.
Mostly A’s

Your vision is balanced and comfortable

You are not experiencing significant clinical visual strain.
It is recommended to continue taking care of your eye health and have routine periodic eye exams to make sure your vision remains stable.

At Least Two B’s

Your visual system is under strain

Accumulated fatigue or headaches may indicate that standard solutions are not precisely matched to your needs.
This is a good time to meet Arza for a precisely engineered multifocal fitting that can eliminate the constant switching between glasses and restore moisture to your eyes.

Even One C

A higher level of expertise is required

This may indicate a complex condition – such as suspected corneal irregularity or a focusing difficulty – that standard solutions cannot resolve.
This is the time to meet Arza – the expert to turn to for complex vision cases. Posterior corneal mapping and precise optical planning based on the physics of vision can help restore sharp, clear vision.
